2024-25 School Year Required Forms:

Student Medical Forms:

Medical Forms are mailed to the home address of all registered band and guard families in late April. Medical forms are due Saturday, May 11, 2024 at Start Up Day Registration.

Medical forms are required as they serve as the blanket permission slip for all DVUSD approved Band, Guard, and Orchestra activities for the school year.  If you have not received one, please email cuttime@mrhsbandaides.org with your current mailing address.

Due July 11, 2024:

• Fall Membership Contract and Band Camp Payment $350
• Band Camp Permission Slip
• BOA Flagstaff Permission Slip (Form due July 9, 2024, Payment due Sept. 9, 2024)
• Code of Conduct / Behavior Agreement
• BAND:  Uniform, Shoe, Glove Order Form and Payment*.
• GUARD: Uniform, Shoe, Glove, Warmups Order Form and Payment*.
• *NOTE:  ALL students need to complete the either the Band or Guard Uniform form.
• Mallet Order Form (Front Ensemble ONLY)
• Stick Order Form (Drumline ONLY)
